Drug Dosage:
Accutane is taken daily twice a day with meals unless contraindicated. The capsule is swallowed whole.

Do not chew or crush the capsule. This medication is also taken with a full glass of water or milk. Follow instructions carefully.

The drug dosage may be adjusted according to response to treatment. If the response is good, Accutane does not need to be taken for 15 to 20 weeks.

After taking Accutane, a two month period of rest is done in which drug is not given because in some cases, acne gets better even without the medication.

However, after the two month period and the acne is still severe, a second course of the medication is done. If the patient is still in adolescence, the rest period may be longer.

Alcoholic beverages must be avoided. Accutane must not be shared with anyone who does not need it because there are high risk of birth defects and serious side-effects are still possible.

Drug Class and Mechanism:
Accutane (Isotretinoin) is classified as a retinoid. It reduces oil production in the skin and changes the characteristics of that oil to prevent abnormal skin hardening.
Missed Dose:

It is important to know that taking two doses at once is not good. If a dose of Accutane is missed, it should be taken as soon as possible, but if it is already time for the next dose, skip the previously missed dose then revert back to the dosing schedule.

Storage:

Accutane must be stored at 25 degrees C (77 degrees F). Store this medication away from light, heat, and moisture. Keep it away from children and pets.

Precautions and Warnings:

Do not take Accutane if you are allergic to Accutane or any ingredient of Accutane. Also, do not take Accutane if you are pregnant, planning pregnancy, breast-feeding, or became pregnant during the course therapy. If any of these are applicable to you, contact your health care provider.

Important:

  • Do not to drive or do anything that requires alertness until the side effects of the medication are known.

  • Do not have any blood transfusions while and even after a month after taking Accutane.

  • Drinking alcoholic beverages should be avoided.

  • Acne worsening may occur at the first part of the therapy, but this is expected, so stopping treatment is not necessary.

  • Use a lip balm or lip moisturizer to prevent lip cracking.

  • Accutane increases chances of scarring so cosmetic procedures such as skin softening, dermabrasion, waxing or laser procedures are avoided.

  • Accutane causes photosensitivity. Avoid tanning booths, sunlamps, and over exposure to the sun until reaction to the medication is known. Sunscreen or any protective equipments must be worn.

  • Health care provider must be informed of any physical activity during therapy.

  • Some birth control pills such as progestin that does not contain estrogen may not be as effective when taking Accutane.

  • Women that are sexually active and of childbearing age should use two forms of birth control at least a month prior to starting therapy, during, and one month after the medication had been stopped.

  • Diabetic patients must check their blood sugar levels often as Accutane can affect blood sugar. Inform health care provider if diabetes is present.

Inform your health care provider if any of these side effects become bothersome:

  • Abnormal swensation of the skin

  • Abnormal growth of hair

  • Redness, bleeding, or swelling of the gums

  • Menstrual changes

  • Chapped lips

  • Dryness of the eyes and mouth

  • Dizziness

  • Dry nasal mucosa and/or nosebleeds

  • Dry skin

  • Fatigue

  • Flushing

  • General body discomfort

  • Headache

  • Thinning of the hair

  • Itchiness

  • Excessive sweating

  • Voice changes

  • Nervousness

  • Anergia



Medical attention must be sought immediately for these severe side effects:

  • Severe allergic reactions (tightness of the chest, difficulty of breathing, swelling of the tongue, mouth, lips, or face)

  • Aggressive or violent behavior

  • Pounding in the chest

  • Dark urine

  • Depression

  • Difficulty or pain during swallowing

  • Difficulty in moving

  • Excessive thirst

  • Fainting of black outs

  • Fever

  • Fracture or weak bones

  • Hearing problems; tinnitus

  • Joint or back pain

  • Leg swelling

  • Muscle weakness

  • Tachycardia

  • Heartburn

  • Severe diarrhea

  • Severe headache

  • Skin infection

  • Slurred speech

  • Stomach pain

  • Shortness of breath

  • Seizures

  • Stunted growth

  • Increased photosensitivity

  • Increase ICP (nausea, vomiting, visual disturbances, increase IOP)

  • Rectal bleeding

  • Red patches on the skin or bruising

  • Swollen glands

  • Depression

  • Vision changes

  • Vomiting

  • Icteric Sclera

  • Weakness

  • Pancreatitis (fever, tachycardia, nausea, vomiting, stomach tenderness)
